Requirements for Admission

State Certification Preparatory Program to Become A Certified Nurse Assistant (CNA I)

1. Physical Examination: Examinations must be dated within the last 12 months and written on HT & CC’s Physical Exam Form. Pregnant applicants must obtain a separate statement from OB-GYN that she is allowed to take the course.

2. Hepatitis B Vaccination or Waiver: Proof of Hepatitis B Vaccination may be indicated by your Physician on the HT&CC, Inc. Physical Examination Form. Vaccine Administration Records may also be submitted as proof of vaccination.  If you have elected not to take the Hapatitis B Series than the participant will need to fill out a Waiver which will be provided by HT&CC, Inc.  or  you may click on the the digital form here for the Hepatitis B  Vaccination Waiver: HEP B Waiver-updated

3. Tuberculosis Clearance:

a)Two steps PPD or skin test is required and must be current (within the year). This applies when your skin test has expired. In addition, all two step skin tests must be taken at least one week apart.
Note: If there is one current skin test, one more skin test must be taken.

Or current chest x-ray (within the year), if your skin test is positive. Please submit positive result of skin test. A written statement from your doctor that you had a positive PPD will meet this requirement. This statement must be written before chest x-ray is taken.

b) If you are pregnant or have a special condition and are unable to take a chest x-ray because your skin test is positive, present a copy of your skin test and obtain a statement from your doctor that the chest x-ray is contraindicated but you have no signs or symptoms of tuberculosis. Obtain a special form from HT&CC’s Clinical Coordinator for this purpose.

3. Picture ID

For e.g., State ID or Driver’s License.

5. Applicants must be at least 18 years of age or have a Parent/Guardian consent.

Patient Care Technician Training (CNA II)

1. Physical Examination: Examinations must be dated within the last 12 months and written on HT & CC’s Physical Exam Form. Pregnant applicants must obtain a separate statement from OB-GYN that she is allowed to take the course.

National Certification Preparatory Program to Become a Certified Phlebotomist

1. Must be at least 18 years old.

2. Must have a High School Diploma.

3. Preferably a CNA.

6. Must have current Adult CPR First Aid Certification.
