How do I Apply for Honolulu Programs?

To sign up for one of our programs you will need to do the following:

For Honolulu Programs:

OPTION 1: (IN-PERSON ENROLLMENT)
1. Come into our office between Monday – Friday (8:00AM – 4:30PM) and meet with an HT&CC Representative.

2. Once there, you will be asked to Fill out out applications and other forms.
3. Please Bring in a valid picture ID. E.g., Driver’s license, State I.D., etc.
4. To reserve your seat into a program, you will be asked to put down a $300.00 Deposit. (We accept cash, credit cards, personal checks, money order, agency purchase order, or cashier’s checks.)

OPTION 2: (ENROLLMENT VIA E-MAIL)
1. Email an HT&CC, INC. Representative at htcc4u@aol.com and attach your completed General Application Form to the email. Click the digital form below to download our General Application form: General.Application.updated
Note: Please include the following information in your e-mail:
1. Program/ Class Title (E.g., CNA Training, Phlebotomy Course, PCT Course, or etc.)
2. Dates of Program you would like to attend. (For CNA: Specify Day or Eve Course)
3. Program Package (E.g., PACKAGE A, B, or C)
4. If applicable, Preferred Uniform Size. (Sizes available: XX-Small, X-Small, Small, Medium, Large, X-Large, XX-Large, or XXX-Large)

2. Once we receive your application an HT&CC, INC. representative will follow up with further instructions and our Accounting department will then e-mail you an electronic invoice which you may pay with a credit card for the required $300.00 deposit to reserve your seat into a program.
Note : Enrollment into a program will be confirmed once we receive all of the required application forms and verification that the required deposit has been recieved.
